Your Video Health Visit with the Matrix clinician will begin shortly.
To prepare for your visit with the Matrix clinician, please:
- Gather your prescription bottles and lab results.
- Have your health insurance card available.
- Have the names and phone numbers of your doctors handy.
- Write down any questions you may have.
- Try to have your height and weight. If you have your blood pressure, that is a big plus – but we understand if you won’t have that.
After your visit, we’ll send you and your doctor your Personal Health Summary. Together, you can make informed decisions about your health.
